Connect Your Devices
If you use an insulin pump or continuous glucose monitor, you’ll receive instructions for securely sharing your data before your visit.

Come As You Are
There’s nothing you need to have figured out before your appointment. We’ll review your information together, talk through your concerns, and determine the best path forward.
WHAT TO BRING
Please bring a list of current medications, any recent lab work/imaging from the last 6 months, diabetic devices if applicable, and your insurance card/form of payment.
